A.B., the above-named plaintiff states as follows:-

On the________ day of ________19______________, the plaintiff and defendant mutually agreed tat the plaintiff should employ the defendant at an [annual] salary of ____________rupees, and that the defendant should serve the plaintiff as [as artist] for the term of [one year].

The plaintiff has always been ready and willing to perform his part of the agreement [and on the _________day of________ 19_______, offered so to do].

The defendant [entered upon] the service of the plaintiff on the above-mentioned day, but afterwards on the _________day of _________19________, he refused to serve the plaintiff as afore said.
