Dear Sirs,

Whereas Shri _________________________ son of late Shri ________________________ resident of ________________________________ was having a fixed Deposit No________________________ dated ____________________.. for _______________. years ________ % interest per annum and the said fixed deposit has become due on _____________ The said Shri __________________________ had not nominated anybody to receive the proceeds of the said Fixed Deposit Receipt on his death under the Banking Companies (Nomination) Rules, 1985.

And Whereas the said Shri ________________________________________. expired on ____________________.. leaving Smt ________________________________________.. and his minor son Master ____________________ as his only legal heirs in accordance with Hindu Law, by which the said Shri ___________________________ was governed at the time of his death.
