hereby give you notice of my claim for compensation for loss as required by Section 78-B of the Indian Railways Act. 

(1) That I booked a consignment consisting of ________. . package from __________, a station on 

__________Rly., to __________, a station on __________Rly., on __________under R. R. No ________. . 

dated __________ 

(2) That I was the consignor and consignee both under the said R. R. of the said consignment. 

(3) That at the time of delivery at __________ station ________ packages were not delivered at all while __________ packages had been badly damaged which caused loss to me. 

(4) That I hereby make a claim for compensation for loss due to short delivery/non-delivery of __________ packages and for damage to __________packages. 

I claim a sum of Rs __________ for the same. 

(The notice can also be given by adopting another Form, viz. notice on behalf of AA __________under Section 78-B, Railways Act for compensation for loss __________ 

If it is intended to adopt the above Form, necessary changes in Paras 1 to 4 should be incorporated.)
